Step 1: Fill out the Schedule an Appointment form above.

This will let us know that you’re interested in becoming an official member of the practice.
You’ll be prompted to choose your preferred physician, name, phone number, DOB, & email, as well as some additional information about your history. This helps us better understand where you’re coming from with your health.
The form will ask for your credit card information, but there are no charges until your first appointment.
We’re then notified that you’d like to be contacted.

Step 2: We’ll give you a call.

This is a free consultation “meet and greet” about our practice, rather than a medical appointment.
Think of it as just a chat so that you can ask any questions you have about our model, what to expect, etc., and decide if we‘re a good fit for you and your family.

Step 3: We’ll schedule you for an in-person appointment to establish care.
